It will default to the lowest difficulty. In other words, if you vox on medium but tar on expert, you'll only get the medium acheivement for both.

I learned this one the hard way.

Try not singing in too high of an octave, or you might have trouble hitting the one or two high notes later in the song.

As people have said, it's whatever feels most comfortable. Try both and see what works. I'm not much of a drummer, but I do heel-up because I find it a LOT easier than heel-down.
